These files are made available for use in your SAR operations. Please feel free to pass them on to your team personnel or direct them to the Crockett SAR website. Please take a moment to read the "Downloads ReadMe" before downloading files. The ReadMe contains some important printing information since a number of the files are to scale. If you pass files on to team members, be sure to include the ReadMe file. This page is broken down into three sections: "Support Software", "Operations Files","Map Tools, Overlays, and Other Transparents"

Blessed Dementia Scale (PDF)An excellent tool to determine the level of dementia of an Alzheimer's subject. This is designed to be answered by the relative or caregiver with the best knowledge of the subject's behavior. Requires Adobe Acrobat or Acrobat Reader.
Forms - ICSThese forms are courtesy of Bay Area Search and Rescue Council. Both standard and SAR adapted ICS forms are available at this site. (My personal favorite for ICS forms)
Forms - Task Assignment (PDF)BASARC's Task Assignment Form updated to correct for two missing words on the one available from their site. Requires Adobe Acrobat or Acrobat Reader.

Forms - Urban Interview Log (PDF)An Urban Interview Log which combines the best of two log sheets I have come across. This log is FANTASTIC for door-to-door canvassing. Requires Adobe Acrobat or Acrobat Reader.
24 Hour Gear (PDF)Equipment list for a 24 hour ready pack. This is the list of equipment recognized for a NASAR SAR Tech II. At the end of the list are some of my personal recommendations/additions. Please note - if you are preparing to take the SAR Tech II exam, ALWAYS consult the official NASAR SAR Tech standards for the current gear list. Requires Adobe Acrobat or Acrobat Reader.
Hydration (PDF)"Suck It Up!" A very informative article on hydration, effects, and prevention. Obtained from National Outdoor Leadership School (NOLS). Requires Adobe Acrobat or Acrobat Reader.
Lost Person Questionnaire (PDF)Full length version - 6 pages. Requires Adobe Acrobat or Acrobat Reader.
Lost Person Questionnaire - Search Data Only (PDF)A 2 page questionnaire that contains only the questions from the full length version that are "Search Data". It is recommended that you utilize this as the first questions asked during the initial interview. This provides for more rapid deployment of teams, since the full length questionairre mixes search data and planning data throughout. Since this version was designed by simply cutting out the planning data questions, it is easy to cross reference with the full length version above. Requires Adobe Acrobat or Acrobat Reader.
Pace Card Creatorby Crockett SAR. This Microsoft Excel template creates pace cards for both U.S. and Metric distances from either your 100 foot or 100 meter pace count. Very useful for providing all members of your SAR team with a pace card. (This file requires the "Analysis Toolpak" be loaded in Excel in order for it to auto-calculate your pace. The "Analysis Toolpak" is loaded via the "Add-ins" menu item in the "Tools" menu of Excel.) (File is Zipped)
PODSheet v2.0by BASARC. PODSheet is a Microsoft Excel workbook that contains several useful spreadsheets for search and rescue operations management. This worksheet based planning program calculates searcher spacing, POD, influence of clues, etc. Download includes instructions and each worksheet has instructions directly on sheet.
SAR Flagging Procedure (PDF)Standard Operating Procedure (SOP) covering the use of flagging tape and light sticks for marking routes and clues during SAR operations. Use of a standard procedure assures consistency and efficiency during multi-agency searches. (This is the procedure currently utilized by the Howard County Fire & Rescue Special Operations Team and the Howard Squadron of the Civil Air Patrol) Requires Adobe Acrobat or Acrobat Reader.

360 Protractor / Compass Rose (PDF)360 degree protractor. This is the preferred square style. I always found the square ones more useful in that they are easier to align with north lines on maps, provide a straight edge, etc. Requires Adobe Acrobat or Acrobat Reader.
Compass Training Aid (PDF)Compass with major components identified. Makes a good overhead transparency as well as a student handout when printed on paper.
ICS SAR Map Symbols (PDF)ICS Symbols for use on SAR maps. Symbols for PLS, clues, resource type used in a segment, facilities, etc. Adopted by NASAR. Requires Adobe Acrobat or Acrobat Reader.
Latitude/Longitude Rulers (PDF)These rulers allow you to determine your GPS coordinates on any 1:24,000 [7.5 minute] map. Requires Adobe Acrobat or Acrobat Reader.
Acres (PDF)Acreage overlay for 1:24,000 scale [7.5 minute] maps. Requires Adobe Acrobat or Acrobat Reader.
Miles - Small (PDF)Mileage overlay for 1:24,000 scale [7.5 minute] maps. (This version is for printers that require 1/2 inch margin.) Requires Adobe Acrobat or Acrobat Reader.
Miles - Large (PDF)Mileage overlay for 1:24,000 scale [7.5 minute] maps. (This version is for printers that can print with 1/4 inch margins or edge-to-edge.) Requires Adobe Acrobat or Acrobat Reader.
UTM Grid Readers 1:24k - Deluxe (PDF)For determining/identifying Universal Transverse Mercator (UTM) coordinates. Use with UTM gridded 7.5 minute maps or with the 1:24k Grid Overlay available below. This "Deluxe" grid reader has the exclusive feature of miles and feet scales along the sides. Requires Adobe Acrobat or Acrobat Reader.
Grid Overlay - 1:24k (PDF)UTM Grid map overlay - horizontal/landscape layout. For use with 1:24,000 scale [7.5 minute] maps that do not have UTM grid lines drawn on them. When using, should be aligned with the UTM system on map, allowing command post to easily convert overlay grid coordinates to map UTM coordinates. Requires Adobe Acrobat or Acrobat Reader. IMPORTANT PRINTING NOTE: Download then open from within Acrobat. Make sure page set-up is NOT set to "fit to page" and IS set to print 100%. As a check for printer accuracy, each grid square should measure 1-5/8 inches.
